Written report or affidavit by an appraiser that includes the date and purpose of the appraisal, a description of the property and all structures, street address, zoning, assessed valuation and taxes, highest and best use of the property, an appraisal method discussion, a history of the property and the area, the appraisers licenses and qualifications, assumptions and limitations, and final value estimate of the property.
